"Cash left in deal" is total cash invested minus cash pulled out at refinance. When it's zero or negative, the refinance returned all — or more than all — of your capital, so cash-on-cash return shows as infinite. The initial loan is treated as interest-only during the hold, so its payoff at refinance equals the original loan amount.
